It's called genital aplasia. There are people born literally with no genitalia or even any semblance or hint of a penis or vagina. I have seen this with my own two eyes. As for urinating, there is an ostomy. It's a surgical opening in the abdomen on which the person wears a bag for urine collection. I've also seen that this person is missing a gluteal cleft (butt crack) and no anus. A colostomy is made for fecal elimination.

A male who has been castrated, meaning his testicles have been removed, is called a Eunuch.

There is a circumcision rite done by African tribes called Infibulation, in which the clitoris and labia are cut out and sewn together, leaving a small hole for urination. As opposed to castration, when the female is ready for marriage, her husband must cut through the stitchings.

There are some people withe 5-Alpha-Reductase-Deficiency (5-ARD) that are unable to convert testosterone to dihydrotestosterone (DHT). Thus, they're testicles may be undescended and have hypospadias, meaning that their penis resembles a larger clitoris. Females may get what is called Complete Androgen Insensitivity Syndrome (CAIS) where the uterus, cervix, and ovaries are non-existent. The vagina may also be shortened and be almost nearly absent. There are also females born with a Fused Labia, in which the vagina is sealed by skin. Though, this is corrected through surgery at birth.
