She died multiple times, but the final time she and Gabrielle went to Japan where she had to fight the army of a ghost named Yodoshi, the Lord of the Darklands. When Xena went to Japan years before she met a girl named Akemi and taught her the Pinch, which Akemi used on her father when Xena brought her home to collect a ransom. Akemi's father was Yodoshi, and he was consuming the souls of everybody who had died in Japan and growing stronger while he destroyed the city and whatnot.
After Akemi killed her father, she killed herself and made Xena promise to spread her ashes in Akemi's family shrine. While trying to take her ashes there in the middle of a storm, Higuchi's citizens (Higuchi is the city this takes place in) form a mob with torches and whatnot to stop Xena because placing Akemi there was soem type of insult. Xena fought back and did her fire spitting thing with a torch and some alcohol and the whole town burned, killing 10,000 people. Xena had to die to be able to take Yodoshi down and Akemi told her if she didn't stay dead the 10,000 souls that died at Higuchi wouldn't be at peace.
As to in detail how she died, she went up against 1,000 of Yodoshi's men with arrows being shot at her. When she approached the leader of the army, I forgot his name, he decapitated her. (She allowed herself to be killed to take down Yodoshi)
Xena had to end because the show had run out of options. She had faced warlords and over come them. Her past had been corrected and she killed the Gods. There was nothing left for her to accomplish other than going from town to town righting wrongs and that would have been boring. There was no more "big" villans left to conquer.
Last Episode in Season 6, "A Friend In Need Part 2". She was killed by a samurai warrior in China to repay a debt from her past.
