Yes, Mammoth Cave was a hospital, a TB Hosptial in fact. Dr. John Croghan, owner of Mammoth Cave, along with everyone else at that time, had no idea how to cure TB. He thought that maybe the cave's constant temperature and humidity would help TB patients. So, he took 16 voluntary patients into Mammoth Cave where they remained for 10 months. They stayed in stone and wooden huts which were built by the slaves of Dr. Croghan. One of the patients left on his own, and eventually most of the patients died. We now know, of course, that the cold, dark, damp cave was the absolute worst place for TB patients, but Dr. Croghan was just trying anything to help his patients.

Taking "largest" to mean longest... Not easy to establish - different sources quote different lengths but the most extensive is the Flint Ridge - Mammoth Cave system, in Kentucky, with over 350 miles of surveyed passages. Caves over 100 miles long are unusual; one of this magnitude rare. This length is notof a single passage but a complex network.
