You capitalize "Dad" if dad is used as a proper noun, "Dad, are we going to the ballgame?" However, dad is lowercase when used as a simple noun or as an example of dads in general, as in "My dad and I are going to the ballgame." The same is true of "Mom is the greatest!" or "her mom bakes cakes."

At the beginning of a sentence, yes. According to The Chicago Manual of Style, 16th Edition, "Kinship names" are capitalized when used in the place of a personal name (unless in apposition). Also, they would be capitalized at the beginning of dialogue.

Examples of correct usage:

It's nice to see you, Son.

I love you, Daughter.

It's nice to see my son.

I love my daughter.

He said, "Son, I'm not as dumb as you think I am."

He answered, "Daughter, I'm sorry."

Do you capitalize the word Bar Mitsvah in a sentence?

Capitalize "mitzvah," but not "bar" (unless it is at the beginning of a sentence). Mitzvah means "covenant" and refers to a specific covenant, and so is a proper noun. "Bar" means son, and since it is referring to any son in general is just a plain old normal noun, and needs no capitalization.
