Depending on if you are talking about the anime or the manga, she gets her powers at a different time.
In the manga, she appeared in Act 23: Saisei NEVER ENDING, which is one of the final acts of the second arc (equivalent to Sailor Moon R). She transforms following the death of Sailor Pluto, when she cries over her friend and her tear transforms into the 30th Century Silver Crystal, similarly to how the 20th century Silver Crystal was revealed when Princess Serenity cried over Tuxedo Mask's fallen form in the first (Dark Kingdom) arc of the manga.
In the anime, she appeared in episode 103 (96 in the English dub), "The Arrival of the Tiny Pretty Senshi!" ("Hello, Sailor Mini Moon" in the dub), during the third season (Sailor Moon S).
By this point in the anime, the location of the lost 30th century Silver Crystal had already been revealed, but not its ability to be used as Chibiusa's transformation item, allowing her to become a Sailor Senshi. Instead, she returned shortly after the start of the Sailor Moon S arc, bearing a letter indicating that she was to be trained as a Sailor Senshi in the 20th century with the other Sailor Senshi. Unlike the manga, her powers in the anime were still in flux, and frequently didn't work as expected. Once she powered up in the SuperS season, her powers became more stable.
Pegasus leaves Rini in Sailor Moon episode 157 (English dub episode 150, "Dream Believer") because he is unable to tell her more about himself, either because of the curse he is under or because it would put Chibiusa in danger. However, Super Sailor Moon helps restore Sailor Chibimoon's faith in herself and Pegasus once more, and Chibimoon is able to summon Pegasus in battle at the end of the episode.
Technically, she dies in Season 5 (Sailor Stars) (which was never dubbed in English), but Sailor Moon defeats the big baddie, Sailor Galaxia, and all the people who died (including Chibimoon) come back to life, kind of like what happened in the climax of Season 1 (Sailor Moon Original/Classic).

It should be around in the middle of the fourth season, Sailor Moon Super S, when Sailor Moon and Sailor Mini Moon are stuck in some sort of dome/force field. Their last powers are also given to them by Pegasus. With The exception of Sailor Moon of course, she receives her last powers/attacks in the last and fifth season, Sailor Stars.

Not in reality, but in your imagination, about anything is possible. Just remember this; not all things that are believable, are true. So, no, humans cannot be a sailor scout like in Sailor Moon, and have super powers.
Out of all the manga villains, it could be argued that Master Pharaoh 90 is the only one that "killed" Sailor Moon when she jumped into the abyss with the Holy Grail after Mistress 9 merged with it. However, she later appeared as Super Sailor Moon, despite the other Sailor Senshi having used up all of their powers (which is what Sailor Moon normally required to become Super Sailor Moon). That and the fact that Sailor Chibimoon did not disappear after Sailor Moon's actions indicate that she was not actually dead, but gravely wounded until she reawakened and healed. No other villain in the Sailor Moon manga could be said to have killed Sailor Moon; they were defeated (by her or others) before they could kill her, or she healed them. In the anime, however, Sailor Moon is either killed or sacrifices herself multiple times to save Earth and her loved ones, so in the anime, it could be said that Queen Metallia "killed" her (at the same time that Princess Serenity/Sailor Moon destroyed Metallia), though Sailor Moon was reborn by wishing on the Silver Crystal.

I do know that of course, the protagonist Sailor Moon is soldier of the moon, and Sailor Chibimoon (or in the English dub, Sailor Minimoon) is the younger version. Isn't there a Sailor Chibi Chibi/ Chibi Chibi Moon in Stars? I can't remember if she's a Sailor Senshi in the anime, but in the manga, I'm sure of it.
