When i pull my foreskin and touch the top part it pains i tried to leave the top part open but when it touches my underwear it starts paining again. is that normal?

No, it is not normal to feel pain from your foreskin. My advice is go and see your doctor. Don't be embarrassed about showing him or her your penis, they've seen plenty of others before, it is part of their job. It is better to have a red face for a minute or two and get treatment or reassurance than to delay seeing a doctor. Failure to see your doctor could at best cause unnecessary worry and at worse risk permanent damage to your penis. I think the above answerer missed the point ! The questioner asks when he pulls back his foreskin and touches the gland/tip it pains! This is normal for boy's who are new to pulling it back. The area of the gland is very sensitive and if it is mostly hidden under the foreskin then it will not desensitize. The gland should be exposed daily through regular pulling back-n-forth of the skin from early age to allow it to desensitize naturally. If this has not happened or the boy has a tight foreskin and is unable to retract it naturally then it will be painful. Ask the doctor for some pain removing gel or spray to help desensitize the gland and try and allow it to be out more. You will get used to it and not notice it after a while.
