Co-transport is when molecules or ions are transported across a membrane (in fixed ratios) at the same time. One example is the Na/Ca exchanger - which transports Na in and Ca out of the cell simultaneously.
One example is a slurry such as those used to transport coal or by Syncor to transport petroleum coke. Another would be sedimentation of river water to form a colloid which transports mud or clay downstream.

Physical and chemical erosion/weathering, followed by transport and finally sedimentation. Transport is not always occuring, it is possible that eroded particles are sedimentated in situ. Also, it is higly likely that during transportation, eroded particles are even further significantly eroded/weathered. For example, when a boulder is transported downstream, it's roundness increases with time/distance travelled.
Upstream and downstream refers to the supply chain of an industry. For example, if you're looking at a distribution plant then the manufacture would be downstream and the retailer would be upstream. Events or processes that happen before whatever is being looked at is downstream and whatever happens after is upstream.
