At regular appointments (how regular depends on which dentist you use. It will typically be quarterly or semi-annually) or if there is something wrong with their teeth or any other part of their mouth.

Well, that's not normal, I think you should bring you 4 year old to a dentist ASAP
He should be 48 months old or simply 4 year old.

Four years old is very young to be losing a tooth. I suppose a dry socket or some other kind of post-operative infection can occur. In any case, the child should be followed very closely by their dentist until it is completely healed.
Two year old children should not be placed with four year old children in preschool due to developmental differences. Two year olds and four year olds usually are learning different things.
