Geralt of Rivia was created in 1993.
His original birth name is simply Geralt. He assumed the fuller name of 'Geralt of Rivia' during his Witcher training, he is not actually from Rivia, but was urged to give himself a second name to make his name seem more trustworthy.
His true home location is not known. While his title is 'Geralt of Rivia', it is revealed that the young Witchers were encouraged to make up surnames for themselves, he is not really Rivian, though has the accent.
