Technical: it is still being investigated but what they think happened is a safety device called a blowout preventer failed to stop the flow of the oil from the well. Somewhere between 9:45 to 9:55 pm on April 20, the well surged and blew out. The oil first was coming out of the rotary hose, and splashing down on the rig floor. At this point, oil was undoubtedly being spilled into the gulf as the blowout preventer failed to activate.

IF you mean the plug actually blew out of the engine head, then I'm only aware of two possible causes:The plug was not fully screwed into the opening in the head.The plug was improperly installed, and resulted in all of the threads either in the head, or on the plug, were totally "stripped-out."
