The official release date is set for September 25, 2012.
On August 28, it is planned for patch 5.0 to hit. This patch is the pre-mop release, meaning it will contain all the talent changes, spell changes and other things like that. When MOP hits, it will release ALL changes and access to all new content and levels will be granted.
You will no longer have to upgrade an account that has Mists of Pandaria attached to it, until Blizzard makes a new expansion. Then you will have to upgrade the account to that expansion.
