Like all previous expansions to World of Warcraft, Blizzard has not released a date for the Cataclysm expansion.
Sure, they might release in November like some people say, but you also have to remember that they are making some HUGE last minute changes to the game (ie major talent point system over haul, major class mechanics overhaul to some classes like hunter and warlock).
They also have not done much work on the new secondary profession Archeology.
It is just better to say that it will be released when Blizzard feels it is ready, or in other words.....Soon.

First and foremost, to become a Worgen, you need to have an account that is paid up to Cataclysm. If you did not purchase the Cataclysm expansion, then you can not be a Worgen.
If you have all of the previous expansions, you can log into your battle.net account, find your WoW game management and digitally upgrade to Cataclysm. Or you can buy the expansion pack at a store.
Depends on what kind of addons they are. You don't need Cataclysm for alot of them, though.
The next version of WOW named "Cataclysm"will be out sometime in 2010. Date has yet to be determined.

No, Cataclysm is currently still in Beta, although some aspects such as the UI have been released to the PTR for testing. This will be part of patch 4.0, before Cataclysm.
