I'm no expert, but when I twisted mine, It didn't hurt. Not even when I lay on them for the first time at night, I think my ears are super sensitive, but super strong! It might hurt the first few times for you though, but this is only because your ear isn't used to the earrings, so it is irritated, but the ear should become use to it after the first couple of times!

Make sure you clean it with either antiseptic or warm salt water twice a day, and with clean hands twist the stud around throughout the day. Trust me on this one, I have my ears pierced twice on the lobe and three times on the cartilage (at the top and middle of my right & the bottom bit of cartilage on the left) and I swim 3 times a week for a club.
