To get there you must go to Port Sarim, talk to Captain Tobias, Seaman Thresnor or Seaman Lorris, and pay a fare of 30gp. They will take you to Karamja, once there head west until you reach the bar, then head north until you reach the ocean where you can then fish for swordfish with a harpoon and a fishing level of at least 50.
