To be honest- there is no such system as far as I know. There is such a system for chemicals (IUPAC), and there is even one for the nomenclature of pharmacologically active substances, but not for diseases- that priveledge is left with the people who either first describe the disease, or with major professional seminars where doctors get together and rename a disease/syndrome, e.g. Mongolism > Down syndrome > Trisomy 21
