Answer: One must remember that Sweeney Todd returned to London illegally, and therefore could not parade around using his old name, which Judge Turpin would immediatly recognise. Sweeney Todd was just a name he chose to use while residing in London.

Yes, there was a real murderer from Italy who was a little bit similar to Sweeney Todd.Leonarda Cianciulli was an Italian serial killer. She made teacakes out of her victims and fed them to her guests. Similarly Sweeney Todd and his accomplice, Mrs Lovett, baked his victims into pies and fed them to customers.
At the end Sweeney Todd is left sitting with his wife (Lucy) who turned out to have poisoned herself, but not died. He is not really paying attention to what is going on around him (He seems to be alone since Ms. Lovett died) and Toby comes out of hiding and sees the razor and cuts Sweeney Todd's throat. I am playing Toby in Sweeney Todd and I know this, I also saw the Broadway show, the movie, and the show on tour. And in all of them this is the ending. Ms. Lovett does not kill Sweeney Todd because Sweeney Todd already killed her.

In the musical/movie Sweeney Todd: The Demon Barber of Fleet Street, Sweeney kills his victims by slitting their throats when shaving them, and disposing of the bodies by sending them down a chute that leads to a bakehouse. From there, his accomplice Mrs. Lovett bakes the corpses into meat pies and sells them to unsuspecting customers.
