We actually don't know. Its origins are lost in time - but it is quite old. You can find it in use at least as early as 1380 in the the text Controversial Tracts by John Wyclif (who is particularly famous for the "Wyclif Bible" - a translation of The Bible into common English published a few years after the aforementioned 1380 text)
