There are two ways to get bodyguards.
First, you can get them by completing a job. The job becomes available on level 18 (Hitman). The job is "Protect your City against a Rival Family" and requires 35 Energy. You don't get a Bodyguard every time, it's random and can be quite frustrating when you burn through all your energy in an attempt to get one.
Second, you can loot a Bodyguard by winning a fight with another mafia. This too is random and can bring on undesired results (other mafia's attacking you and taking hits out on you). You have to win them as loot against other mafias.
