The Blood Knight Insignia can be found on a dead blood elf in Deatholme.
Blood of the Wrathful is obtained by summoning a level 20 elite demon in Razorfen Kraul, by clicking on an orb near Jergosh the Invoker.
Crate of Bloodforged Ingots is inside a clickable crate near the stables in Shadowfang Keep.
Corrupted Kor Gems can be looted from Nagas outside Blackfathom Deeps. Note, these are the ones in the tunnel outside the instance, you do not need to go inside.
