Salazar Slytherin secretly built the Chamber of Secrets and hid a Basilisk inside after the other founders refused to only accept students Slytherin seemed worthy. The chamber was sealed and could only be opened by a Parselmouth. When that student came along he would be able to release the snake and purge the school of muggle-born students.
Lord Voldemort was the only person to ever open the chamber, once in 1952 and again in 1992. The second time he was possessing and acting through Ginny Weasley.
The entrance to the Chamber of Secrets is inside the second floor girls bathroom. One of the taps, which has never worked, has a small snake on it. Speaking parseltounge will open the entrance.

In 1942 the Chamber of Secrets was opened. Several muggle-born students were petrified and one died, Moaning Myrtle. After her death, Hogwarts was to be shut down. Tom Riddle framed Rubeus Hagrid and his pet Acromantula, Aragog. Hagrid was expelled and Hogwarts remained open. Tom Riddle then left part of his soul in the diary so that one day, the Chamber could be opened again.
