There are two methods for obtaining the Blood Pact (deed) for Death. The first method is to simply progress through the story missions. The last one has you seking out Reyes in Escalera. Not going to spoil it for you but after the final story mission (called "On a Pale Horse") you are simply given Death. The second method involves the "Four Horses of the Apocalypse" challenge. After you find and break War, Pestilence AND Famine, you will randomly encounter Death as a blue circe like the other 3. Unfortunately, there is no location that has a better chance of spawning Death than any other, it's completely random. I have heard that you must have found and killed the one and only Chupacabra before Death will spawn but I don't know if that's true. Once you break Death and achieve rank 4 on the challenge, simply use Death's ability to kill one each of the four zombie types (Fresh, Retcher, Bolter and Bruiser) and you'll attain Legendary rank for that challenge, and you will receive the Blood Pacts for ALL FOUR horses. Best advice if you want to find Death is to let some time pass by saving or warping around, and keep checking places you would have found the other 3 horses. Good Luck!

The only 5 horses are war-fire horse ;pestilence-horse with green stuff ;famine-skinny horse but faster than all horses except unicorn ; death- a horse with white stuff around it and you will find out his special ability when you get him ;but last the unicorn- has butterflies around it and have a rainbow behind it.
