It's in Undernet 2. The skull is near the Graveyard and the Undernet BBS. The big holes are the eyes, the little holes are the nose, and the BBS and Graveyard is the horns. In the middle of the eyes, press A (only works if you're doing the Ghost Navi Request) a couple of times and the ghost navi should appear.
