The protagonist fell of a yacht and ended up stranded on an unnamed island in the Caribbean, where he became the prety for the antagonist. If I remember correctly (and you should probably verify this, as it has literally been decades since I've read it), the protagonist was American and the antagonist was European.. perhaps Russian?
