According a recent visit to Toyota dealership, it appears that the fuel filter is located inside the fuel pump. That is, you need to remove the fuel pump in order to change the fuel filter wich is part of the full assembly.

If they told you full service included the filter change and you agreed to full service then you agreed to a filter change and you had a meeting of the minds and a verbal agreement. If they did not change the filter they have broken the agreement.

follow your fuel lines and find filter hold the fuel lines make sure to not break or crimp line and use a wrench to remove filter make sure there is no flames anywhere near cause there is gas in the filter
Assuming you mean the full-size vans, it's on the fuel pump in the tank.
It shouldn't. You should probably replace your fuel filter (which is inside the fuel tank).
