Goblins and Worgen will not be getting their own capital cities. Instead they will be getting "disctrics" in their factions capital city (Orgimmar or Stormwind). They will be getting their own starting zones (possibly phased like the DK starting zone).
This is a question that can not be answered at this time. This is because Cataclysm is not out yet and we do not have enough information to make a decision one way or another.
Currently, it is already possible to do an entire character customisation, including race and even faction change. Since this is already available, it will unlikely be a special feature in Cataclysm. It is possible that race changes to a Goblin and Worgen are disabled for some time into Cataclysm, however.
when cataclysm is released, the geography of the original game will change permanently. this means that even if you don't purchase the expansion you'll still be able to see the changes. however, you will not be able to use any new content that comes out with cataclysm, such as the two new races (worgen and goblin)
In World of Warcraft, Worgen is a racial option that becomes available when you have the Cataclysm expansion installed and your account is upgraded to include Cataclysm. When you make a new character, Worgen and Goblin will then be options for you to choose as a race.

If your account is flagged for Wrath of the Lich King, but you did not buy Cataclysm, the game works as normal. When Cataclysm is released, the Worgen starting zone will be Gilneas (In Silverpine). The starting area, along with the Goblin starting area, will be phased like the Death Knight starting area in Wrath of the Lich King.
To get the Cataclysm expansion, simply upgrade your account. Note: To get the Cataclysm expansion, you must first upgrade to Wrath of the Lich King expansion, then upgrade to the Cataclysm expansion.
Worgen and Goblin will be Death Knights, because it says it on the site of Blizzard. They just might not be available right away when Cataclysm is released. This is due to Death Knights having a head start in the leveling process.
