NOT SURE ON YOUR 2004, BUT MY 2000 IT IS LOCATED IN THE CARGO AREA ON THE DRIVERS SIDE. THERE IS A REMOVABLE PANEL NEAR THE HATCH. THE CONTROL MODULE IS LOCATED JUST BEYOND IT AND HAS A STICKER WITH THE CODE # ON IT.

I believe the module is located in the console above the rear-view mirror.
It is located in three places. First, On the keypad entry code identification card (credit card), found with the owner's manual at the time of sale, which appearantly you do not have. Second, in the memory of the keyless entry module, which the dealer can retrieve for you with a scan tool, and third, on a sticker located on the keyless entry module, the location of the module depends on what year Windstar you have.
codes on my 2001 were located behind the passenger side rear seat seat belt panel. I got instructions off EBay for $15. Well worth it.
there is a "wallet card" which has the code on it, if you do not have this card you will need to bring the car into a Mercury dealer and they will "plug it in" and retrieve the factory preset code.

It is located on the left side of the driver seat floorboard. The trunk release lever also functions as the keyless entry module if it is pushed down.
The keyless entry module on a1998 Honda Accord is located underneath the dashboard. It is housed on the driver-side the of dash close to the main relay.
It is on the computer module, which is located the the trunk, on the driver's side, in the space compartment with your spare tire tools. The computer module is a white box, and the 5-digit number is located on the top of the module ( you will need a flashlight and a flat-head screwdriver to pry the panel to see it ). I pushed the backseat down to stretch out and see the top of the module easier. You will see other smaller numbers on the the top of the module at first, but keep looking near the back, right, area of the module and you will see 5 large numbers ( those are the numbers you are looking for ) Also, if you want to create you own code, type in the 5 digit factory code, then press the 1-2 button, followed by a 5 digit code of your choice. Do Not make all 5 numbers the same! e.g. 11111, 12121, ect. After you type in your 5 digits, open the door, lock all doors, and test. (When i set my passcode, no lights appeared or horn honked, but it did work). After you are able to use you key-less entry system, you will be able to do other options with it.
