To have a piercing "off" (removed), it's best to go back to the shop you got it done & have the piercing artist who did it remove it. If something is wrong or goes wrong, chances are they can handle it there.

CA state law requires the shop to have a consent form or the person be over 18 for a tattoo or piercing. When you go into a tattoo shop in CA the shop has required forms for you to fill out. One of the items required is your driver license number and you sign the form that what you have listed is correct.
