I'm not positive about all the states, but I'll take a shot at it. I suspect that the five states are: Texas, Oklahoma, Kansas, Colorado and New Mexico, with the two "panhandle" states being Texas and Oklahoma.

Mexico, angered over the annexation of Texas, attacked Texas settlers in what was by now a US State. As a result, Polk was able to persuade Congress to declare war. After a decisive victory, the United States gained 500,000 square miles of territory including Pacific regions that are now modern day San Diego and San Francisco.
