Many moderns allow laypeople to receive Communion in the hand and distribute it at Mass or on sick calls, a very novel practice that has become widespread from the generous interpretation of a vague point in the New Code of Canon Law. This open interpretation is resisted by more Conservative elements in the Church that continue to only allow the Host to be handled by one with consecrated hands.

Priests administer the sacraments of Baptism, Reconciliation (confession), the Holy Eucharist, and Anointing of the Sick. They officiate at Matrimony (although it is the couple themselves who actually administer the sacrament to each other). Holy Orders is reserved to bishops only, who are the normal ministers of Confirmation as well. However, priests can serve as the ministers of Confirmation with permission of the bishop.
All of the sacraments allow believers to experience God's grace. When sins are confessed and forgiven, believers receive God's grace.
The Second Vatican Council addressed Sacraments, but it did not define them, they had previously been defined. Vatican II addressed the fact that Jesus was present in the sacraments, the He was the source of the sacraments, it gave directives for communal celebration, the Eastern Churches, indulgences for the sacraments, norms for administration of them, the purpose, sacred music, and addressed the revision of them, but it did not define them.
The sacraments that you are referring to are Christian elements. Given the nature of the celebration, Jewish people do not do this. This is a peculiarly Christian thing and is not found in Judaism.
