It's hard to say. At first everyone thinks it's Anna, because of the way she talks about only living in relation to her sister, which we know from just the back of the book it how Anna felt. But when you read the book and see that (Spoiler alert!) Anna dies at the end, then reread the prologue, your opinion changes.
If you look at it piece by piece, you see that it actually seems to be Kate that is talking. First, it says that the speaker is 3 years old, which means if it was Kate speaking then Anna would have been an infant. This makes the line "She didn't stand a chance against me, of course," make more sense. (If it were Anna speaking, Kate would have been about six and was healthy at the time, and could have easily fought her sister off.)
Next, it says that as the two grew older, the speaker only existed in relation to her sister. While this seems like it comes from Anna's point of view, think about how Kate felt in the book about what her disease was doing to the family. It is possible that she had felt that way since childhood, and had wanted to die all along so her family could be happy. She knew that Anna was saving her life so she must have realized that killing Anna would, in turn, kill her too. It says that the speaker thinks about this for several years, which is certainly possible, because by the time she relapsed at age eight, she was old enough to see the strain it caused her family.
The biggest clue comes at the end, when it is revealed that the speaker's sister died. We know that Anna is the one who died, and that Kate blamed herself for asking her to file the lawsuit. This guilt is shown in the lines, "In the end, though, I did not kill my sister. She did it all on her own. Or at least this is what I tell myself." After Anna's death, Kate probably tried to convince herself that it was Anna's fault for agreeing to file the lawsuit or for not backing out. She tells herself this to keep herself from feeling guilty, though it most likely doesn't work, as in the epilogue Kate openly says that she blames herself.
For these reasons, I think it is Kate speaking in the prologue. It is also reasonable that if Kate speaks only in the epilogue, she should speak in the prologue too, and if you pay close attention the prologue and epilogue are written in the same font (each character had their own distinct font).
In the book 'My Sister's Keeper' by Jodi Picoult, the character who is narrating the prologue is Anna Fitzgerald. She provides a glimpse into her life and the difficult decisions she has had to make.
