By definition, a transgender is one, such as an author who will be pretend to be the opposite sex (in the case of an author, to keep their identity a secret). By today's society though, a transgender would be considered the same as a transsexual, one who feels they have been born the wrong sex. If you hear of a MTF transgender, then that means they are a biological (or born) male, who wants, and will go through surgery, take hormones, or just crossdress in general to make themselves a women. Typically a trans (short for transgender) is born this way, and they prefer to be refered to as the gender they want to be, not the one they are born. It is considered one of the most painful things for one to have to live with, for they constantly feel trapped inside a body with no exit. It feels like getting stuck in a very small hole with no elbow room, and no way to get out for your entire life. And society will laugh at you for being in this hole. They will look at you, and they will prevent you from coming out as you scratch the walls screaming for freedom that you can't get to. But I hope that answers your question.
