Apollo 14 was launched on January 31, 1971. This mission was the third crewed mission to land on the moon and was commanded by astronaut Alan Shepard.
Apollo 12, the second moon landing mission, launched on November 17th 1969 (it landed on the moon on November 19).
Apollo 13 was the spacecraft that had an oxygen tank explosion, not the lunar module that landed on the moon. The lunar module that landed on the moon during the Apollo 11 mission was called the Eagle, piloted by astronauts Neil Armstrong and Buzz Aldrin.

There were two "pilots" on board; Michael Collins was the command module pilot, Buzz Aldrin was the lunar module pilot. However, in practise all three astronauts were pilots - Neil Armstrong actually piloted the lunar module onto the moon's surface.
The lunar module used during the Apollo 14 mission was named "Antares".
