Brenda Lee-1972
Brenda Lee made the first recording of "You Were Always On My Mind".
The first answer always counts.
"You Were on My Mind" sung first by Ian and Sylvia in 1964 and covered many times during the decade. "Georgia on My Mind" was written by Hoagy Carmichael in the 1930s and made a hit by Ray Charles in 1960. "Always on My Mind" was a song from the 1970s famously recorded by Elvis in 1972.
Elvis recorded it in 1972, the year after it was written. Nelson's version was more than a decade later. Actually the first recording was done by Brenda Lee. Lee's came out in June, Elvis's not until November.

It depends. If somebody was previously someone you used to always think of, it is "were". If you still always think of them, use "are" "You are always on my mind." This means he/she is still on your mind. "You were always on my mind." This means he/she is not on your mind any more.
It was probably first recorded by the Babylonians who were the first known astronomers, in about 600 BC.
