There were hundreds different rulers in the kingdoms spread throughout the medieval period in Europe. Most were ruled by kings, but the republic of Venice was ruled by the Doge, The Holy Roman Empire by an emperor, and the Duchy of Milan was ruled by a duke.
There was no one, all time ruler in the Middle Ages. That period lasted for roughly a thousand years, and the countries that existed changes substantially in identity and in amount of territory controlled throughout the period.
