Assuming this is about the Emancipation Proclamation:

Technically, no one. When Lincoln gave the Emancipation Proclamation, it "freed" the slaves of the Confederacy, composed of rebellious states that had seceded from the Union. However, because they were no longer part of the Union and under Lincoln's leadership, the Proclamation technically had no effect on them at all. He also excluded the border slave states that were still part of the Union from the Proclamation because, if they were to secede in retaliation, the Union would be destroyed. Lincoln knew all of this, and so he made the Emancipation Proclamation as more of a symbolic move than an actual law.

What document stated that Slaves of any state in rebellion shall be then thenceforward and forever free?

The document that stated "Slaves of any state in rebellion shall be then thenceforward and forever free" is the Emancipation Proclamation. Issued by President Abraham Lincoln on January 1, 1863, it declared the freedom of all enslaved people in Confederate-held territory during the American Civil War. This executive order was a significant step towards the abolition of slavery in the United States.

What did the battle of Antietam did?

It led immediately to Lincoln's Emancipation Proclamation, declaring all Southern slaves to be 'thenceforward and forever free', and turning it into a war on slavery. This would prevent the British from helping the Confederates (which they were planning to do) because it would make them look pro-slavery. Lincoln had had to wait all summer for a Northern victory that would give him the credibility to make this announcement.

Lincolns decision to free the slaves?

That was the Emancipation Proclamation, which declared Southern slaves to be 'thenceforward and forever free', but only in the states that Lincoln did not control! (He allowed slavery to continue in the four slave-states that had remained loyal.) Lincoln was not actually deciding to free the slaves. He was making it look like that, so that free nations abroad could not aid the Confederates without looking pro-slavery themselves.
