The first known activities that seem to be related to religion and/or spirituality (or more specifically, attention paid to an individual after his/her death) can be traced back as far as 60,000 years ago, and easily more. Religious activities are suspected to be even older, possibly beginning as far as 300,000 years ago. At that time, skeletons of Cro-Magnan man, Neanderthals, and eary Homo Sapiens were found neatly buried underground with crude, handmade tools laid nearby. It is unclear what started this trend, but it is well-documented that it occured.

There are artifacts and other such evidence that idol worship (cause unknown) dominated the many years after, until the rise of the Abrahamic religions that follow the partiarch Abraham, who, according to legend and certain religious texts (including the Holy Bible and the Qu'ran), fathered the beginnings of Isreal. According to Judeo-Christian belief, he also copied down the word of God into the five books that compose the Torah (the first five of The Bible; also the basis of Jewish Law).

To summarize:

The earliest explanations for religions and/or religious activity can likely be attributed to the earliest intelligent considerations for the unknown of death. The departed were first placed in the ground to prevent their use as food for scavengers; then they were placed in the care of idols and/or living people who assumed their thrones under pretense of supernatural ability or preference. Much more recently, religions find their origin by claims of the word of God (i.e. divine inspiration).

The reasoning for such would be specific to each religion. But as a general rule, 'obedience' should suffice (whether to a leader, god, or God).

If you are referring to all the religions of the world, then the answer is no. The answer to the first question I put as "sort -of" because many of the original protestants started out as Christians in the Catholic Church but left the Church to start their own religions, which they then labeled as "Christian not Catholic." In actuality, they were "heretics not Christians." The 30,000 denominations today are mostly offshoots of those original protestants or their offshoots. Since Martin Luther, Calvin, and Henry VIII started their own "churches" in the 16th century, everyone is trying to get into the act and start their own.
