I would say that both the wealthy and diseased people were the ones who mist suffered. I believe this because without the peasant the wealthy would not earn money and without money they couldn't buy foood or water or anyhting else. For the peasants who were diseased suffered the most because of the plague, there are 3 types of plague: Bubonic, Septcemic, and Pneumonic. These can be received from rats and especially fleas. The plague would give them fevers and abdominal pain, this would also enlarge lymph nodes which are bumps that are visible outside your skin when you have the disease. Therefore my answer who suffered the most is mostly the peasants.
