Hamlet is the last person to actually die in the play. However, Rosencrantz and Guildenstern are the last people to have been mentioned dead--by Fortinbras I believe.

The theme of Hamlet's second soliloquy is his main internal conflict. Hamlet wants revenge fort his father, but dislikes the idea of exacting revenge. Hamlet becomes frenzied and anxious during the second soliloquy, trying to resolve this conflict.
Hamlet is directly responsible for the deaths of five people and indirectly for another. The first person to die because of Hamlet's actions is Polonius, who Hamlet stabs in the mistaken belief that he is King Claudius. This indirectly causes Ophelia's death. Hamlet then kills, in succession, Rosencrantz, Guildenstern, Laertes and Claudius.
Usually it's Laertes. However, a lot depends on how the director chooses to stage it. In one production, Fortinbras's army lays hold of Horatio and put him up against the wall. Fortinbras says, "Go, bid the soldiers shoot" and they shoot Horatio. In that production Hamlet was the second last to die.
