False. At the beginning of the play, Bernardo and Francisco also saw the ghost. They were the first ones stated to have seen the ghost of Hamlet's father.
No in the opening scene Horatio, Hamlets friend, sees the ghost, along with two sentries, Marcellus and Bernardo. The Sentries have seen the ghost on previous nights and have called Horatio to see it too.

MacBeth ends up seeing the ghost of his friend Banquo during his dinner party he throws in (and double check this) Act 4 i believe. Also, before MacBeth kills Duncan in Act 2 he sees a floating dagger as a hallucination. The ghost though, is Banquo.
Which time? In Act 1 he was waiting for the ghost to show up, hoping he would talk to him. In Act 3, Hamlet is haranguing his mother when the ghost turns up (well at least Hamlet sees and hears him).

Laertes talks with his father, then he leaves the room shortly after Ophelia enters Hamlet is conflicted, brooding, and resentful when he sees his mother Gertrude becomes upset with Hamlet because Hamlet does not like Claudius..
Gertrude is quite shocked by Hamlet's confrontation. Not only did he kill Polonius in the room, but Hamlet also goes into a rage of criticism against his mother and sees the ghost, which Gertrude does not see.
