It all depends on the size of the heel, the shape of the shoe, and the size of the shoe.
Wearing a size to small or pointy shoes with too high of a heel can cause hammer toe, corns and callouses. The higher the heel the more pressure on the toes and the ball of the foot. Constantly wearing high heels of the same size of heel for many years can cause the tenden and calf muscle to shorten, making it not possible to walk flat footed.
Heels can be worn without injury if the proper size and shape is worn. Not all feet can wear pointy shoes. A box end or a more rounded toe can be more comfortable. The right shoe could be comfortable all day and evening.
The heel is also important. A size 5 shoe can have a higher proporitonal heel than a size 9 of the same still shoe. The smaller size could have a steeper angle for the arch than larger sizes, but it varies with the manufacturer. A 4 inch heel may only be 3.5 inch in a size 5 but 4.5 inch in a size 11 for some manufacturer. While others may use the same heels for all sizes. So a size 5 will be higher for the foot than a size 11 with the same heel.
Also rotate the heel size thru the week. This keep the chance of Calf shrinkage down and allows the foot to rest from the higher heels.
There are no serious direct health issues related to high heel shoes, although they might cause sore feet/bunions by putting pressure on the front of your feet, plus some women have difficulty walking in them, which means there is a higher chance of tripping and twisting your ankle. Finally they can be very distracting to men who will be more liable to not see where they're going.
