All twelve of them in the Western calendar have 28 days - though some have more. Some countries / religions used lunar or other calendars in which months may have a different number of days.
All calendars are correct. Some are more precise than others, or more fully reflective of the seasonal year.
It depends on the civilisation. Some civilisations had a calendar just to suit their own cultures, while some more advanced civilisations actually created accurate calendars based on astronomy, just like the ones we use today.
