There are two ways you wont get the skull achievement even when you pick up the skull. Either you are playing the mission on easy or you started from a rally point. There are also skullls on maps, but you can only get to them in forge. And if you need help finding all of the skulls, you can just search "Halo 3 skulls" on YouTube and the top three or four are helpful. I hoped this helped you find other skulls.
