Medicine cats can't fall in love because it is against their beliefs in starclan. They also are following what other medicine cats did in the past. Cats have been known to break these rules though like Leafpool and Yellowfang.

Medicine cats cannot fall in love because they must remain impartial and focused on their duties to their clan.
