Are you talking about the manual 5 speed or the automatic trans? I you're asking about the 5 speed, it could be that the "shifter guide" in the top of the transmission box is either worn out or dislodged. A $5 part with a $250 labor bill. Yikes!

1) reduce throttle to idle speed 2) move gear shifter to Neutral position 3) move gear shifter to Reverse position.

are you serious? yeah you do- the O/D switch is on the side of the shifter- and if you're asking this I'm assuming you have an automatic....'cause a manual- well yeah you have to shift.
There is a lever just to the right of the driver's seat left of the e-brake/shifter. You have to move the lever to the right and up to get it out of 4 wheel drive.
That depends what year is the 4 runner?
Remove the access plate on the floor to expose the trans. disconnect the transfer case shifter as 2 of it's bolts are shared with the shifter on the trans. The bracket is held on to the 2 left hand shifter bolts and will have nuts on the top to remove, it is not possible to just move it out of the way without removing the other 2 bracket bolts further down on the trans. Then remove the 4 retaining bolts on the top of the shifter and pull it out.
remove access plate on floor around shifter, 4 bolts hold the shifter onto the trans.
Check the door jam on the drivers side of the 4 Runner you will most likely find a sticker there that tells you the right tires for your 4 Runner.
