Where was owain glyn dwr born?

He is said to have been born at his residence in Glyndyfrdwy near Corwen in North East Wales, at what was probably his principal residence at Sycharth near Llansilin on the Wales / Shropshire border or least likely at Trefgarnowen in Pembrokeshire where his mother was from. Another intriguing possibility is that he was born at Rhuddallt (now Wynnstay Park) near Ruabon, Wrexham. See The Last Mab Darogan: The Life and Times of Owain Glyn Dwr' for full details of where and when he was born.

Did Owain Glyndwr die on the mountains?

No one knows. He was an outlaw on the run from Henry V when he died so it's entirely possible he died in some remote place. Tradition says that he died at one of his daughters' houses (either Grosmont or Monnington being the choice of many) but this seems unlikely as they almost all lived in England or at least close to the English border. That he died on Lawton's Hope Hill in Herefordshire (even further into England) was not recorded until as late as 1513 (in an English translation of Tito Livio of Forli's Life of Henry V) so it's more likely to be more myth than fact. You could read more in 'The Last Mab Darogan: the Life and Times of Owain Glyn Dwr' by Charles Parry pp.305-8.
