God appeared to Abraham at least three times in the Scriptures: Genesis 12:7, Genesis 17:1, and Genesis 18:1. Each of these appearances was significant in the unfolding of God's covenant with Abraham and the promise of a great nation.

According to tradition, God first spoke to Abraham when he was in his seventies. This answers the question: since Abraham had already been serving God intensely for many decades, God then spoke to him (not the other way around).

Note: Genesis 12:2 and 12:7 are two separate prophecies. The same goes for 15:1 and 15:7 according to Jewish tradition; though others may not be aware of this.
However, Genesis 15:13 and 15:18 may or may not be counted as one prophecy together with the preceding verses. In ch.17, verse 15 may or may not be a prophecy distinct from the earlier verses. In ch.18, verses 26-32 may or may not be a continuation of the prophecy in verse 1. And in ch.22, see verses 11 and 15 (in which an angel speaks, not the Lord directly).

According to tradition (and this will come as a surprise to some) the covenant in Genesis 15 occurred before the events in Genesis 12-14. Abraham was 70 years old at the time; he traveled briefly to Canaan and then returned to Terah in Harran until he reached the age of 75. Traditionally, Abraham was 70 years old in 1743 BCE. That (Genesis 15) was his first prophecy.
