David Cameron, or any UK Prime Minister, is the head of government of the United Kingdom of Great Britain and Northern Ireland. This includes England, Scotland, Wales and Northern Ireland. However, Scotland has its own government and Wales and Northern Ireland have their own assemblies. So those constituent countries have their own "leaders" while England has no devolved government.
Scotland established its own Parliament in 1999, following a referendum in 1997 that saw a majority of voters supporting the establishment of a devolved Scottish Parliament.

Prior to the 1707 Acts of Union Scotland was a kingdom and had its own monarchy, although since 1603 the King of Scots had also been the King of England. Wales was not a kingdom.

Scotland and England were kingdoms in their own right, Wales used to be called a principality and Northern Ireland a province. Ireland is both a state and an island, the island comprises of the state of Ireland and that part of the UK called Northern Ireland.
