Evaporation is the vaporization that takes place only on the surface of a liquid. Evaporation takes up energy - and it takes up that energy from the warm surface of your skin. Thus, when sweat evaporates, it takes heat from your skin, cooling it.

In physical chemistry, the transformation of liquids to vapors is called an endothermic process, meaning it requires or absorbs heat to effect the change. There are other physical and chemical changes that give off or emit heat and these are called "exothermic". The evaporation of sweat is, essentially, the transformation of a body fluid to a vapor and, therefore, absorbs heat from the body.

When a liquid shifts phases from a liquid to a gas, energy is required. Think of the molecule of liquid sitting in a puddle. If the molecule wants to jump off of the puddle and become a free gas molecule, it will need energy to push it out of the puddle. It takes energy in the form of heat energy from your body to do this when your sweat evaporates. The net effect is that you are cooled off. The faster a liquid evaporates, the more energy it absorbs. This is why a more volatile substance, such as rubbing alcohol, feels even cooler when it evaporates off of your skin.

As sweat on the surface of the skin heats up, it gains energy. When sweat droplets evaporate, the heat contained with them is removed, thus cooling the skin.
