As new cells grow the old cells fall off. You normally don't notice this since they simply fall off your body and are too small to see. However since the foreskin covers the penis head, these cells are trapped inside and accumulate to form smegma. Smegma can be prevented by simply pulling back the foreskin and washing the penis head with soap and warm water in the bath or shower.

While this is not completely true, it's not completely false. A more accurate statement is that uncircumcised men are more likely to have smegma under their foreskin than men who are circumcised.

A yeast (candidal) infection of the skin or a mucous membrane, such as the mouth, often produces a white cheesy material at the infection site. This type of infection, known as thrush.
Sounds like smegma. It's not altogether unhealthy, but is definitely unhygienic and unattractive. Make sure you clean under there at least daily.

This is normal in children, as skin cells from the inside of the foreskin fall off and migrate to the opening of the foreskin. Small white bumps along the ridge (corona) of the penis head are also normal. They are callled "pearly penile papules" and occur in about 1 out of 20 men.
