No, not always.
This means that you o2 sensor is sensing a lean fuel mixture (rich gas) so to correct this problem start by changing your o2 sensors, checking you spark plugs to see if they have white tips, replacing your pvc and cleaning your air intake with carb cleaner (if it is stock)
Mine has 4 O2 sensors, one before, and one after the CAT on each bank. (just a FYI, I was getting an O2 lean error caused by the short flex pipe just after the Y leaking)

If you have not changed the Oxygen sensors I would do that. The O2 sensors ( which are a maintance item) need to be replaced when you have incorrect fuel delivery problems. Mark I had the same codes on my 96 Silverado and it was my fuel pump. Usually your 02 sensors will have a {no activity},{no response} or {heater circuit malfunction} when they are bad.
All O2 sensors are threaded into the exhaust system at various points. Upstream O2 sensors are located before the catalytic converter(s) and downstream O2 sensors are located after the catalytic converter(s).
